RLF Permission to Treat Form- All students going on any off-campus youth activity must fill out this form in full and return to their youth leader.

2010 Membership Form - If you are interested in becoming a member of RLF, download our membership form and return it to the Kiosk. Remember, we renew memberships annually.

Background Check Form - If you are interested in helping with our nursery on Sunday mornings, Great! However, to protect our kids and volunteers, we ask everyone working with kids to submit a background check. Please download this form and return to the RLF office.
